How can I View and Download Government Forms?

            In Salarium, you can generate the following Government Forms:

            For Companies

            BIR 1604-CF - This is the Annual Information Return of Income Taxes Withheld on Compensation and Final Withholding Taxes. For information on generating the 1604-CF, please click here.

            BIR 1601-C - This is the Monthly Remittance Return of Income Taxes Withheld on Compensation

            Note: Tax forms that companies need to fill out are now digitally filed and submitted through the Online eBIR Forms System as required by the Bureau of Internal Revenue (BIR).  Based on the Revenue Memorandum Circular No. 14-2015, tax payers must use eBIR forms, which can be downloaded from the BIR website, and can be used when submitting BIR Form 1601-C, or the Monthly Remittance Return of Income Taxes Withheld on Compensation.

            RF-1 (Philhealth)- This is the Monthly Employer's Remittance Report

            R-5 (SSS) - This is the Monthly Employer Contributions Payment Return

            R-3 (SSS) - This is the Quarterly SSS Contribution Collection List

            HQP-PFF-053 (HDMF) -  This is the Monthly Member's Contribution Remittance Form

            Note: Before exporting government forms, make sure you have completed payroll generation, and have closed the payroll. For more information on how to generate payroll, click here.

            To export government forms, click Payroll at the top of the page.

            Click Government Forms. You will see the various government forms that have been generated from payroll generation.

            Click the government forms you want to download. 

            For Employees

            BIR 2316 -  This is the Certificate of Compensation Payment/Tax Withheld. This is generated annually, or when an employee resigns or is terminated.

            To download an employee's 2316, go to Payroll>Government Forms>Employees and click BIR 2316 in the Download section.
