Generate SSS Forms

R3 and R5 SSS Forms


What is SSS R3 Form?

This form contains the list of employees and their SSS contributions deducted from their salaries for a month to be submitted to SSS.


What is SSS R5?

This form is used for when making payments to SSS. It contains the following information:

  • Applicable Period (Month)

  • SS Contribution (Total amount of Employees’ SS contributions)

  • EC Contribution (Total amount of Employees’ Compensation contributions)

  • Total (SS Contribution + EC Contribution)



Generate SSS R3 and R5 Form

To Generate SSS Monthly Contribution R3 and R5:

1. On the Top Navigation, click Payroll > click Government forms.


2. Click the Generate Government Form button and select SSS.


3. Under "Select which form you would like to generate” drop down, select SSS Monthly Contribution R3 and R5 Forms


3. Under “Select the month” drop down, select the Month


4. Click the Continue button


5. Once the form has been generated, fill out the "Total Amount Paid in Words" and “Document Number” (Payment Reference Number - PRN)


6. To download the form, click the Save and Download button
